Fox Valley Package

starting at $995

Direct Cremation is for families who want a simple and flexible farewell without a viewing or service.

Package Includes:

Professional services of a licensed funeral home
Local removal from place of death by licensed staff
Filing of all required paperwork by the State of WI to complete the death certificate and required cremation permit from the county of death
The cremation process
Obituary posting on the Cremation Society of the Fox Valley website
Black Plastic Cremation Urn
Minimum Cardboard Cremation Container

*Does Not Include: Visitation, viewing, or ceremony, cash advances such as permits or third party fees, death certificates, unlisted additional services, unlisted merchandise, or sales tax.

**Additional fees may apply if more than one person is required for the transportation, if there are steps into a facility, if removal is required outside of regular business hours, or if your loved one is 275 pounds or over.

Medical Assistance Package

starting at $1,895

This package is for direct cremation with no services. This WFCAP State Burial Assistance if approved will be paid in full by the State of Wisconsin Burial Assistance Program. No charges to the family if approved.

Basic services of funeral director & staff
Crematory fee
Shipping of cremated remains
Black Plastic Cremation Urn
Minimum Cardboard Cremation Container
Five (5) Death Certificates

*Please submit all the information so we can relay that to the state or approval or denial.

*Does Not Include: Visitation, viewing, or ceremony, cash advances such as permits or third party fees, obituary publication charges, unlisted death certificates, unlisted additional services, unlisted merchandise, or sales tax.

*Please note that you must have confirmation and approval from the State of WI. One of our funeral directors will take the name of the deceased family member, date of birth, confirmed date of death and social security number to confirm or deny eligibility. To select this package. Please call us at 920-593-1000 to confirm eligibility. If eligible, the cost of the cremation and transportation will be covered by the state, and the cremation permit fee may be waived from the medical examiner as this varies from county to county.
